This form is to request that a player be moved to a higher or lower division than is stated by their birthday.
example: a player born in 2011 would normally be in the "13U" division according to the birth year. This request form would be submitted to
have that player moved up to the "15U" division where players are born 2009-2010. The same would apply if you were moving a 2010
born player down to the "13U" division. Both these situations need to be approved by the Yorkton Minor Baseball board of directors.
